This package Includes a step by step guide and timeline along with all necessary documents to successfully submit your application, including:

·        Quality Assurance Forms (for both tracking and compiling findings):

This package will include everything you need to successfully implement the mandated quality assurance program within your Lab.  The directions and timeline will guide you through the process to a successful application submission.

PACKAGE 2:

Consultant Accreditation Services:

$2500.00

Do you need a little more assistance than just forms and templates, but feel capable of gathering data necessary for the quality assurance program?  If so, this package is for you and in addition to Package 1 includes:

·        Explanation of standards, guidelines and  IAC process

·        Evaluation of the Labs current practices by a Consultant

·        Recommendations and identification of areas for improvement

·        Consultant personalizes the Policies and Procedures

·        Consultant  compiles all QA findings gathered by your Lab

·        Consultant prepares the PowerPoint presentation for the first mandated QA Meeting

·        Review of application and attachments prior to submission

This package is intended for the Lab that has the time to contribute to the gathering of Quality Assurance Date, but needs some assistance with personalizing policies and procedures, compiling the QA findings and preparing the QA meeting.  You will have the added comfort of knowing your application and all attachments will be thoroughly reviewed prior to submission.

PACKAGE 3:

Full Service Consultant Accreditation Services:

$3500+ (size and location of Lab will affect price)

Are you too busy to get started and complete the Accreditation process?  Not enough time to gather QA data, and complete the electronic application? Do you need full service assistance?  If so, this package is for you and in addition to Package 2 includes:

·        Consultant will gather the clinical correlation

·        Consultant will participate in the peer review process

·        Consultant will identify and assign physician peer review studies

·        Consultant will complete the electronic application

·        Consultant will add all necessary attachments to application

·        Assist with CME requirements

·        Review Case studies prior to submission for compliance
